The Road to Value-based Care: Disease Recognition
This is a 10-week course that expands your knowledge of value-based care, emphasizing the importance of early disease recognition and proper documentation, and equipping you with tools to navigate from a volume-based to a value-based care practice.
The introductory 2-part module explains the necessity of moving from volume-based care to value-based care and how that transition will benefit the patient, the physician, and the health care system as a whole.
Specific topics covered include the foundation, principles, and science behind the HCC Model, the basis and use of the RAF Score, how value-based contracts work, and the importance of early disease recognition.
The subsequent eight modules focus on specific disease types providing real-life examples of how best to recognize and document conditions including common pitfalls for identifying and recording each disease and key takeaways.

Certification in Value-based Care
This “bootcamp” is a single day session illustrating our philosophy and best practices based on our model of patient-centered care.
Course goals include:
- Clearly demonstrating how a move to a value-based model is beneficial for patients, quality of care, society, and providers.
- Providing a ‘second residency’ explaining how to successfully practice value-based care by sharing teachable skills.
- Sharing what we have learned on our 20+ year journey to help others to become more proficient at a more rapid pace.
Quality Delivery Oversight Committee (QDOC)
Pivtl offers comprehensive consulting services that assists your team in building your own Oversight Committee. We begin our approach by doing a detailed needs analysis followed by recommendations and next steps to help you build and manage a highly effective Oversight Committee.
The QDOC team is responsible for:
- Identifying gaps in care and areas for improvement though the analysis of financial, quality and utilization metrics.
- Overseeing, updating, and educating our physicians for the purpose of improving the care of our patients.
